Grear food and kitchen staff. The new kitchen manager always ask about our likes and dislikes. Love the nurses. Reminder: This is not a hospital. You may or not get the same services.
I want to thank Lebanon center for rehabilitation and health. They went out of their to bring my mom home. Something they. Did not have to do. My mom was very well taken care of there. I went in often. I always felt she was safe and cared for. Thank you again.
My mom was recently discharged from the Lebanon Center for Rehabilitation. We were concerned when mom was released from the hospital about which rehab would be the best. This was the best decision we could have made. The staff were so supportive and friendly. We could not have been happier with her care The entire process was made easy due to the efficiency of the staff and the coordination of nursing, PT, OT and leadership. Thank you for the care and encouragement to mom and us.
One star. A tour that looked nothing like the actual care. We chose this facility for my grandmother's stroke rehabilitation after touring five centers and narrowing to three. During the tour the place felt warm and the staff seemed attentive and kind. The reality after admission was the opposite. On arrival, no staff greeted us or told us what to do. We waited two hours before my mother finally went to the nursing station herself. When she asked the charge nurse, Kim, what paperwork we needed, Kim rolled her eyes, huffed, and explained the forms in a condescending, oversimplified way. My mother is an RN. Only after she said so did Kim's tone change—which tells you the dismissiveness was a choice. While we waited, my grandmother's roommate sat in a soiled diaper for an hour and fifteen minutes despite asking for help three times. Help only came after my mother went into the hall and asked directly. This was not a one-time lapse. On discharge day, a patient called out for help for 32 minutes with no one checking on him. The contradictions were just as troubling. During the tour we asked specifically whether we could take her out for meals and activities and were told yes. On June 5th we took her to a pool, where she only sat with her feet in the water. On June 10th my mother was called and reprimanded for it, told it suggested grandma was "healthy enough" not to be there, and informed she could no longer leave for any reason until discharge—directly contradicting what we'd been promised. Her June 9th care plan also listed her as not a fall risk, which is flatly incorrect and a serious safety error. The worst came at discharge on June 16th. We arrived at 9:36 for a 10:00 readiness time and weren't seen until 10:42. Kim said she would call in grandma's prescriptions and send home the remainder. At home we discovered one medication had been given at double the prescribed dose. A medication error, a misclassified fall risk, patients left calling for help, broken promises, and a charge nurse who was consistently condescending and intimidating. My grandmother looked frightened every time we saw her. The five-star marketing does not match the care. I would not send a loved one here.
This is sort of a story with two chapters. First, the good. The staff was extraordinarily kind. They took care of our family member as though he were one of his own. My thanks to each of them. The physical therapy staff took their time and worked with our 90 year-old family member to help him regain some strength. Their patience and kindness made the process comfortable and effective. Second, the not so great but acceptable. Hospital food is hospital food is hospital food… The food is generally bland and occasionally less than appetizing.Cooking for large numbers of people is always a challenge. The food was acceptable though not outstanding. Overall. We are grateful for the outstanding manner in which they assisted our family at our time of need. Thank you again.
My dad has been a resident here since January. The staff has been wonderful to work with. My dad is almost 92, it does my heart good to see how the CNA’s love on and care for him when I can’t always be there. It’s so hard when you are a caregiver and you realize you just can’t do what they need at home anymore. The administrator and the business office manager have help me and been so supportive. Kim, the day charge nurse is awesome. Her response and attitude to the patients and their family set a great example for her co workers. The planned activities are many and encourage interaction with all resident. Are they always perfect, no. But they are always doing their best and it shows. I am thankful my dad has found a happy place. Even on the hard days with dad the support me & my dad receive are a blessing.
My husband couldn’t walk or stand when we got here. Hospital sent him here for physical therapy. They were fantastic in getting my husband on his feet and walking. Everyone there is super good at their jobs. We are now home and doing well . They showed me how to handle him with his exercises. Place is very clean and tidy. Entertainment for patients during the week really helps also. Would choose this rehabilitation center again. Gwen Krug
Good place for my dad to recover. Clean facility and the staff is friendly. The food serves its purpose and I get updates and notifications often.
