this is a very professional facility. with a lot of caring individuals that took great care of my mother, and make sure she was prepared for her discharge I like to thank you all everybody that I came in contact from the bottle of my heart thank you very much
Walked into pleasantness. Very nice clean and pleasant to the eyes and nose. Traveled from Indianapolis and was very pleased.

This place is horrible! My family member left in worse shape than when she arrived. They just drugged her and put her in a diaper instead of giving her the therapy she went in for. She ended up with a HORRIBLE rash and they just put diaper cream on her. They wouldn't help her to the bathroom...just terrible! The outside of this place is heaven, the inside is hell! Thank god we moved her to a beautiful place called NHC in Gallatin TN. Do NOT leave your loved one here! THE GOOD REVIEWS ON HERE ARE FAKE!! I wish I could leave zero stars but maybe a lawsuit would be best!

I appreciate the love and care that the Creekside staff show to my brother. They treat him as if he is a member of their family. I feel a deep sense of comfort knowing that my brother is under the care of some incredibly amazing people. May God continue to bless the nurses, CNAs, housekeepers, therapist, dietary staff and the entire Creekside village. If you have a love one in need of short or long-term care, visit Creekside.
I just called about giving donations and a couple people that answered the phone were very rude, I wouldn’t send my family member here!
Usually a great facility, do not admit yourself on weekends, they are short staffed, they do not answer call buttons for patients, not attentive at all on the long term side, takes long time to get medication. I’m disappointed in this administration and staff.Night shift your on your own as a patient.
