Definitely putting my mom here

My mother died in this facility back in October of 2018 - I have been devastated with guilt that I ever placed her in this nursing home. She hated it, I was stuck not knowing what to do. She was never taken outside, she thought was in prison. She had COPD and many times her 02 tanks would be empty on the back of her wheelchair. She ate alone in her room often because there was not enough staff to open the dining room. Even her death was handled inappropriate, we were not given a private room, residents were coming in to stare at my dying mother. I was told by the ombudsman to send my mother…

I was told by the ombudsman to send my mother to a hospital that the staff there did not know what they were doing. Do not send our loved one to this place. I regret it every day and I only pray my mom can forgive me.

My Dad has been in this facility twice for rehab. The staff all seemed to care about him (as much as strangers can care for another). We both thought it a much better place than the other area nursing homes he was in.

It’s a very out dated and I didn’t enjoy my time there at all.

After 4 1/2 years, had to move my father. Since Concordia took over, it is unsafe. Policy changes have made it almost negligent. Staff is over worked and understaffed. Even the best and most caring staff are defeated and just trying to do their best.

this place is awful and should be shut down! its a fraud! heres why! 1. my aunt had a stroke that took over the entire right side of her body. because of her stroke she couldnt walk and was bedridden. 2. Because she was bedridden and overweight, she couldn't rotate in bed and needed help to do it. But! Whenever she called for help no one came until an hour or so later. And because of that she ended up having a giant gash on the back of her butt. They also tried to claim that it happened when she was in the hospital but after looking at reviews for this place afterwards she wasnt the only one. 3. These people never tell you that they THROW AWAY YOUR LOVED ONES ITEMS WITHIN 30 DAYS IF THEIR IN THE HOSPITAL TOO LONG. So they lost my aunt's phone and CPAP machine resultimg in us having to pay $200 to replace her phone. 4. Because my aunt couldn't move she couldn't get up to charge her phone and these people kept unplugging it, resulting in her phone being dead. 5. My aunt ended up having delirium and had lost 90% of her memory. She couldn't remember who visited her, what she ate, what day it was, what year it was, or her dog's name. These people tried to take advantage of this by saying that they helped her. That was Bull because at one point when we visited her, her colostomy bag was ripped and there was s*** all over her. 6. We tried to move her out of this place but she wanted to be put in there because SHE thought that they really were taking good care of her. They took advantage of her lack of memory. 7. She ended up having to eat mushy food. So when they tried giving her solid food and she was able to eat it, she was eating it for a week straight when she suddenly was put back on mushy food for no reason. Not even the nurses knew why she was given mushy food again and if the nurses don't know there is no reason she should be on mushy food. 8. And worse yet, they were told to put her on do not resuscitate, but they went behind her family's back and had her sign paperwork that we didn't authorized. So she ended up going to the hospital twice and physically suffer. So if you or your loved ones end up having to go to a nursing home, STEER CLEAR OF THIS PLACE! The place is now called Concordia and it doesn't deserve to be called a nursing home.

You will be very happy here. The staff is very nice and will treat you good.

THIS PLACE DOESN’T EVEN NEED A HALF OF A STAR but I had to in order to leave a review! FOR STARTERS THERE IS NOT ENOUGH ROOM ON HERE FOR ME TO SAY EVERYTHING! My father in law has been here since last Friday. He was in the hospital on life support for almost 8 months from really bad case of pneumonia. After God performed a miracle and got him off the ventilator (everyone said he’s too weak, lungs too damaged, he’ll be on a vent the rest of his life,etc-from every doc & respiratory therapist). After walking 1 step in this place I instantly felt & knew it wasn’t good. I walked to the nurse station to get my father in law a cup of ice chips & the 1st thing I heard from a nurse speaking to the lady who also worked there was, “oh yeah I gotta tell you what my b**** daughter did.” That is very unprofessional and should not be heard discussed at work. The staff look, talk, act, & treat their patients very unprofessional,, RUDE, hateful, and a whole lot more! They do NOT take care of the patients at all! I’m beyond disgusted with this place and whoever owns it either needs to do a major makeover with the entire place including employees and/or get fired or replaced themselves. I WOULDN’T let my dog stay in this place. THIS FACILITY NEEDS TO BE INSPECTED & SHUT DOWN. Very disgusting! Its not the least bit of clean or sanitary. My father in law who’ has a trach in and doesn’t have strength or muscles too stand or walk, doesn’t have hand sanitizer in the room!! They don’t have the stuff to clean his trach and haven’t cleaned it once! They haven’t suctioned him & physical therapy they supposedly have hasn’t came in one time to do a single thing with him! I will be contacting the health department & state of TN to have this place inspected & shutdown! The way they let this place be ran & how they treat people make me sick to my stomach & being in healthcare myself, THIS PLACE IS AN EMBARRASSMENT & disgrace on healthcare in itself! 😡
